Why are we asking?
We are evaluating and improving the existing Pathology Hub educational service for UCSF Pathology trainees. We invite adults who are current UCSF Pathology residents or fellows and are not on the Hub evaluation team. Taking part is optional and does not affect evaluations, employment, rotations, educational opportunities, or access to the Hub. The results are for program improvement and are not being shared outside the evaluation team at this stage.
What can you choose to provide?
The baseline survey asks about your pathology information needs, lookup time, resources, barriers, and training stage. A later follow-up may ask about your experience with the Hub. You may skip questions, review answers, and leave at any time. Your answers stay in this tab until you select Submit survey; leaving or reloading clears unsubmitted answers. The optional comment may contain identifying details, so please do not enter patient, personal, or identifying information, including report text.
The Hub separately offers optional capture of clicks on the broad Books, Journals, and Lectures category links. It starts off on every page load and requires a separate choice for each visit. It does not capture search text or search submissions, diagnoses, topics, individual resources, chat text, drafts, keystrokes, or activity at destination sites. You can stop future click capture with Stop capturing activity. Earlier recorded events remain. An unused visit permission expires after 30 minutes.
What is stored and who can see it?
Survey responses and category-click events are stored separately in restricted PostgreSQL databases on Google Cloud. They are not linked to each other or across survey rounds. Survey records have a random submission ID and a receipt; category events have a random visit ID and an event order. These records do not intentionally contain names, email addresses, IP addresses, or patient information. Free-text comments and uncommon combinations of answers may still identify someone. Cloud providers may retain operational or security logs, including IP information, separately from the evaluation tables.
Charles Herndon is the configured individual-export user. Other program stakeholders receive aggregate summaries with identifying combinations omitted. The proposed retention period is at least three years after evaluation completion, or longer if applicable requirements or holds apply. Automated backups currently retain seven daily copies. Deleting an active record does not immediately remove backup copies.
Can I ask for removal?
Keep the receipt shown after submitting if you may want to request removal. Contact Charlie.Herndon@ucsf.edu with the receipt and without repeating sensitive details. There is no self-service deletion. Without a receipt, the team may be unable to locate a response by name or email. Aggregated results and backups may not be immediately separable. Category-click events are not linked to survey receipts.
